Release of Medical Information
All UMW Student Health Center medical records are retained for 7 years after graduation and then destroyed. If you would like a copy of any portion of your Health Record, please complete the editable PDF file Record Release Authorization. You may type your information directly into the PDF before printing or print the form and fill out by hand.
