New Students
All students with 9 or more credits MUST provide a completed UMW Medical Form to the Health Center. Please make sure we receive your form by August 1st if you are entering the Fall Semester or January 3rd if you are entering the Spring Semester. Submit all forms in the student portal at https://students.umw.edu/healthcenter/
If you fail to submit a form then you will be put on Student Health grade/registration HOLD.
