NEW Women’s Clinic patients: Complete this Women’s Clinic Previsit Questionnaire and follow the directions at the top of the form. After we receive the form, a member of the health center staff will contact you for your appointment.
If you are being scheduled for a gynecological exam, please read First Gynecologic Exam.
RETURNING Women’s Clinic patients: Please call the health center to make an appointment for your three-month pill check or your yearly exam. We no longer require returning appointments to be made in person. Please read the pre-exam instructions.
